We’re looking for an healthy lifestyle practitioner to join our One You Westminster service. The One You Adult Weight Management Practitioner role is a key role in a wider Cardiovascular Disease Prevention Service. The focus of the role will be delivering a range of healthy lifestyle programs including weight management courses and physical activity. The post-holder will work closely with clients to help them achieve their goals and liaise and communicate client progress to their caseload managers and the Healthy Lifestyle Advisers. The role will also include updating and monitoring of client progress using the service database and working with partnership organisations to deliver interventions to target groups. A typical day as a Adult Weight Management Practitioner might include Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values! Delivering evidence-based interventions that focus on diet, nutrition, physical activity, behaviour change, and psychological support. To work with Healthy Lifestyle Advisers to ensure clients are supported and motivated to make behaviour changes through their various interventions with the service. To implement aspects of an individual’s health improvement plan including SMART goal setting taking into account their risk factors, individual lifestyles and motivations. Working collaboratively with the team and partners to create referral routes and meet targets. Ensuring an excellent and safe experience for residents using our policies and procedures. Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources!
